The Application Process
| Step 1: | Call us at (866) SPA-POOL (772-7665) to request a SplashCard application. |

Once you receive an application via US Mail, complete it and mail directly to: POOL SPA USA, 2564 Coquillard Drive, Niles, MI 49120-9507 |
| Step 3: | Pool Spa USA will fax your application to the SplashCard processing center. Within 24-48 hours, we will contact you regarding card approval. |
| Step 4: | Purchase your pool or spa products. |
